restrictive lung disease Flashcards
def. restrictive lung disease
disease that reduces the size of the lung (TLC)
- TLC
3 possible cause
- interstitial lung disease (parenchymal)
- chest wall restriction (extraparrenchymal)
- neuromuscular (extra)
what is ILD
disease that increases the thickness of alveolar membranes
2 effects of ILD
- increased stiffness of lung
2. reduced diffusion across alveoli
4 steps to dx ILD
- Hx
- Phx
- CXR with interstitial changes
- PFT - parenchymal restriction
what is seen on CXR
reticulations and nodularity
what is seen on PFT for ILD
reduced TLC
- proportional reduction in FEV1 and FVC
- both reduced, but FEV/FVC is normal
what is seen in DLCO in ILD
reduced - can’t get O2 across
what is chest wall restriciton
squeezing of the chest wall from the outside
3 possible cause of chest wall restrciotn
- obesity
- bony abnormalities
- pleural disease
what is important measurement for bony abnormalities
cobb angle
>60 - associated with lung restriciton
>100 - can lead to resp. failure
2 types of pleural disease
- effusion
2. calcifications
what is seen in PFT of chest wall restriction
- TLC
4 regions that can cause a neuromuscular disease of lung
- C-spine 3,4,5
- mech
- ischemic
- inflammatory - phrenic nerve
- trauma
- inschemic - NMJ
- myasthenia - diaphragmatic muscle disease
how to make a diagnosis of neuromusc (5)
- Hx
- PHx
- abdominal paradox
- PFT - same as mechanical
- tests of resp. muscle strenght
