Determination of brain death Flashcards
1
Q
what is def. of neuro death
A
irreversible loss of capacity for consciousness combined with irreversible loss of all brainstem functions, including capacity to breath
2
Q
minimum crit. for NDD (5)
A
- established etiology causing death that is irreversible
- deep unresponsive coma
- loss of stem functions and reflexes
- absent resp. effort based on apnea test
- no confounds
3
Q
potential confounding factors
A
- unresuscitated shock
- hypothermia
- severe metabolic disorder - glucose, lytes
- peripheral nerve or muscle dysfunction of NMJ blockade
- drug intoxication
4
Q
what is apnea test
A
give oxygen and pull ventilator - positive is no response for 10 minutes and pCO2
5
Q
when is ancillary test performed
A
- when impossible to complete minimum criteria
2. deep unresponsive coma
6
Q
what is ancillary test
A
demonstration of global absence of intracranial blood flow - CT angio
7
Q
minimum requirments for physician to declare NDD (3)
A
- current license
- skill and knowledge in mgmt of severe brain injury
- no transplant conflicts of interest
8
Q
what are brainstem reflexes tested
A
- pupillary - shine light
- extraoular - doll’s head and caloric reflex
- corneal reflex
- gag - stim back of pharynx
- cough - stim tracheobronchial tree by passing canula into endo tube
9
Q
5 physiological changes in NDD
A
- central diabetes insipidus
- vasodilation
- apnea
- hypothermia
- other endo dysfunction
10
Q
4 diffs between NDD and pesistent veg. state
A
- spontaneous breathing in PVS
- cardio stability
- sleep wake in PVS
- long duration in PVS vs hours to day in NDD
11
Q
what to do after diagnosis
A
- offer organ donation
- complete death cert.
- offer autopsy
- coroner?
