Steps in Tx
A
- quit smoking
- prolongs, but never reverses - B2 agonsists
- long acting salmeterol best - inhaled anti-cholinergist
- ipatropium bromide - combine B2 and anti-chol
- inhaled corticosteroids
- O2
- based on ABG - PaO2

Tx for acute exacerbations
A
- start with dilators
- systemic steroids
- ABx (PNA)
- 02
- BiPAP
- intubate if necc.
