NMJ disoreders Flashcards
what is motor unit
one motor neuron and the muscle fibers it innervates
what happens when AP
all fibers activated
what happens at NMJ
AP>Ca in pre-syn>Ach release>binding to nicotinic>Na channels open>muscle
what is myasthenia gravis
IgG antibody against nicotinic receptor
what do antibodies do (2)
- block Ach binding
2. attract complement for AchR destruction
2 factors related to MA onset
- genetics
2. age > 50 (women get younger)
what is potential origin of autoimmunity
- thymoma in 10%
2. thymus hyperplasia in 70%
4 clinical signs of MA
- fatiguability
- Proximal>distal
- ocular weakness
- bulbar weakness (CN9-12)
what is safety factor
release more Ach than needed for synapse
2 special tests for MA
- edrophonium - AchE inhibitor - should get better
2. ice pack test - cold inhibs AchE
what is MA crisis
resp failure due to muscle weakness
what is lambert eaton disease
IgG antibodies against pre-synaptic Ca channels
what is association with lambert eaton
50% malignancy, 80% ar small cell lung
4 clinical signs in lambert eaton
- post-exercise facilitation
- gradual leg weakness proximal>distal
- autonomic dysfunction
- minimal ocular/bulbar
