hyperkinetic movement disorders Flashcards
what is dytstonia
abnormal sustained, but not fixed posture of a body part due to involuntary muscle activation
what is cause of dystonia
co-contraction of agonsists and antagonists
what is general presentation of dystonia
mobile rather than fixed, often task specific
- can be tricked with sensory tricks
what is dystonic tremor
typically jerky with ireegular frequency and amplitude
what are 2 classifications of dystonia
axis 1 - clinical Sx
axis 2 - etiology
what are axis 1 features (3)
- distribution (focal, multifocal, segmental, general)
- additional feat (task specific, paroxismal)
- age of onset
what are 2 axis 2 types
- primary (idiopathic or genetic)
2. secondary (trauma, stroke, degenerative)
what are primary idiopathic dystonias (3)
blepharospasm, torticolis, task specific (writes cramps
what are genetic dystonias
young onset
DYT1 gene
3 secondary, degenerative/metabolic dystonias
- huntingtons
- wilsons
- PKAN
what is wilsons
AR disoreder of gene mutation causing accumulation of Cu in brain and around eye
what is essential question in dystonia
Is this a secondary dystonia?
what is workup for dystonia (3)
- MRI
- gene testing depending on presentation
- if DYt neg - Cu, trial of DOPA
5 treatments of generalized dystonia
- artane
- baclofen
- DA depletin agent
L-dopa - DBS
treatment for focal dystonia
botox
