Oral Cavity Cancer
starts in mouth
Oropharyngeal Cancer
Starts in oropharynx
Head and neck squamous cell carcinoma (HNSCC)
Term used for cancers of the oral cavity, pharynx and larynx
oral cancer causes (no one knows oral)
exact cause unknown
oral cancer - Predisposing factors
▪ Constant overexposure to sun ▪ Poor oral hygiene
▪ Tobacco use
▪ Chronic ETOH use
▪ Chronic irritation
▪ Syphilis
▪ HSV
▪ HPV
▪ Immunosuppression
Oral Cancer
Clinical Manifestations
Leukoplakia, Erythroplakia, 30% of pts have asymptomatic neck mass, Lip CA usually presents with indurated, painless ulcer, Tongue CA usually presents with tongue ulcer or area of thickening
Leukoplakia (luke = white)
▪ White patch on mouth mucosa or tongue
▪ Precancerous lesion
▪ <15% turn into CA
▪ Patch keritonizes (becomes hard and leathery)
Erythroplakia
▪ Red velvety patch on the mouth or tongue ▪ Precancerous lesion
▪ >50% progress to CA
oral cancer - 30% of pts have
asymptomatic neck mass
Lip CA usually presents with
indurated, painless ulcer
Tongue CA usually presents with
tongue ulcer or area of thickening. ▪ Pain may occur, especially when eating hot or spicy foods
▪Later s/sx
increased salivation, slurred speech, dysphagia, toothache, earache
Oral Cancer Diagnostic Studies
▪ Oral exfoliative cytologic study
▪ Scrapes suspicious lesions for microscopic examination
▪ Toluidine blue test
▪ Test for oral CA
▪ Toluidine blue is taken up by CA cells ▪ CT
▪ MRI ▪ PET
