Metabolic or autoimmune? (sjoin the autoimmune)
It is an autoimmune disease that causes the obstruction of the secretory ducts and glands, typically the salivary and lacrimal glands.
ETIOLOGY/ PATHOPHYSIOLOGY (connect me with sjorns)
Connective tissue disorder
More common among persons with certain tissue types:
HLA-DRW52, HLA-DR3, and HLA-B8.
CLINICAL SX’s of SJOGREN’s - Insufficient tears causes
inflammation and ulceration of the cornea.
SJOGREN’S SYNDROME: DIAGNOSTICS (Shirmer is a sjogren)
Tests to confirm KCS: i.e. Schirmer’s test
Tests to quantify xerostomia: i.e whole sialometry and Saxon test.
Salivary gland biopsy
Serologixc testing : Ro/SSA and Lo/SSA, RF, ANA.
MRI: to evaluate salivary glands.
TREATMENT of SJOGREN’S
No cure: symptomatic management.
Artificial tears
Salagen for dry mouth
vaginal lubricants
Decreased exocrine gland function leads to the “ sicca complex”: a combination of
Dry mouth: Xerostomia
Dry eye: keratoconjunctivis sicca
Other glands that can be affected (sjogren’s is a pins)
stomach, pancreas, intestines, vagina.
Insufficient saliva decreases
Insufficient saliva decreases digestion, promotes tooth decay, and increases nasal and oral infections, epistaxis.
Dry vagina causes
pain during intercourse (dyspareunia)
treatment (Chemo for venus)
Low dose chemotherapy can slow the progression of Sjogren’s by suppressing the immune and inflammatory response (Methotrexate, Cytoxan)s

Corticosteroids, Plaquenil (may slow inflammatory response)
