- Lies in inverted V-shaped space medial to scalenus anterior (vertebral triangle)
- It lies between
- In front: inferior thyroid artery and carotid sheath
- Behind: inferior cervical sympathetic ganglion C7 and C8 nerves

Ascends in foramina transversaria of upper 6 cervical vertebrae surrounded by plexus of veins and sympathetic plexus of nerves

Spinal branches to spinal cord and meninges

Lies in suboccipital triangle grooving upper surface of atlas posterior arch
