Digastric Triangle: Submandibular Salivary Gland Flashcards
Parts
- Superficial
- Deep
Superficial
- Main part
- Superficial to mylohyoid
Superficial: position
Submandibular fossa of mandible, wedged between it and mylohyoid extending:
1. Above: to mylohyoid line
2. Below: to intermediate tendon digastric
3. In front: to mental foramen
4. Behind: to mandible angle
Superficial: surfaces
- Inferolateral: superficial below mandible
- Lateral: deep, in contact with mandible body
- Medial
Inferoateral surface relations
- Skin
- Superficial fascia: contains Platysma
- Deep fascia
- Submandibular lymph nodes
- Anterior facial vein
- Cervical branch facial nerve
Lateral surface relations
- Submandibular fossa
- Medial pterygoid (insertion)
- Facial artery (between gland and medial pterygoid)
Medial surface relations
- Anterior: mylohyoid with nerve to mylohyoid and submental artery
- Intermediate: hypoglossus with 6 structures on its lateral surface
- Posterior:
- styloglossus
- Stylohyoid ligament
- Glossopharyngeal nerve
Deep part of gland and its duct project from
Medial surface of superficial part
Deep
Thin flat tongue like process deep to mylohyoid
Deep position
On Hyoglossus lateral surface with lingual nerve above and hypoglossal nerve below
Submandibular duct extends along it
Site of continuation of superficial and deep parts
Around mylohyoid posterior border
Submandibular duct site
5 cm long arising from medial surface of superficial part running forwards along deep part between:
1. Mylohyoid (superficial)
2. Hyoglossus (deep)
3. Lingual nerve (above)
4. Hypoglossal nerve (below)
At anterior border Hyoglossus, lingual nerve hooks around (double crosses) the duct then continues forwards between:
1. Sublingual (superficial)
2. Genioglossus (deep)
To open in mouth flor in sublingual papilla summit on each side of tongue frenulum
Why is the chances of having a stone in the submandibular duct 50 times higher than the chance of having them on the parotid duct?
- Secretion is thick and viscid
- Ducts drains against gravity
- Ducts opens in mouth floor, food particles may enter forming stone nucleus
- Patients complains of:
- submandibular swelling
- increasing in size during meals
- returns to normal size in between
Atrial supply
Glandular branches of facial artery
Nerve supply
Passing through submandibular ganglion:
1. Sensory: lingual nerve
2. Sympathetic: plexus around facial artery
3. Parasympathetic: chorda tympani
