ophthalmic emergency - usually due to acute trauma such as HBC, fighting, or falling on their head
brachycephalics can proptose an eye with MINIMAL trauma
what are 2 positive prognostic indicators for a potential return to some functional vision in a proptosed eye?
- positive menace response
- positive PLR
what does a dilated, unresponsive pupil in a proptosed eye indicate?
optic nerve avulsion is likely - poor prognostic indicator for return to some level of functional vision
what percentage of dogs with a proptosed eye regain some functional vision?
only 20%
diagnosing a proptosed eye is fairly obvious, so what must be done upon admission?
must evaluate the patient’s stability for surgery & prognosis for the eye/vision
what is the treatment of choice for a dog presenting with a proptosed eye after being HBC?
must STABILIZE & TREAT LIFE-THREATENING INJURIES FIRST!!!!
reposition the globe into the orbit ASAP if vision or globe can be salvaged
if the eye/globe can be salvaged in a dog with a proptosed eye, what are your treatment options?
reposition the globe into the orbit ASAP - lateral canthotomy, replace globe into the orbit, & temporary tarsorrhapy
if you were able to salvage a proptosed eye, why may enucleation still be a possibility for the future?
needs to be done if the eye is painful & blind
if the eye/globe can’t be salvaged in a dog with a proptosed eye, what are your treatment options?
enucleate!!
what are 4 kinds of extensive damage that would indicate the need for enucleation of a proptosed eye?
- globe rupture
- severe intraocular hemorrhage (hyphema)
- avulsion of 3+ extraocular muscles
- optic nerve avulsion
