Fluconazole
Inhibits lanosterol demethylase for fungal cell wall synthesis
Fluconazole considerations
Increases QT interval so cannot be given for heart problems
Abnormal potassium/Mg/Ca levels
Clotrimazole
Inhibits lanosterol demethase for fungal wall synthesis
Clotrimazole considerations
Causes oedema and paraesthesia
Nystatin
Binds to ergosterol and forms a pore in fungi for the leakage of ions
Nystatin considerations
Cannot be given to neonates. Causes diarrhoea and angioedema.
Amphotecerin
Binds to ergosterol and causes pore formation for ion leakage
Amphotecerin considerations
It is nephrotoxic. Causes diarrhoea, anaemia and decreased appetite.
Pyrimethamine
Anti-folate which Inhibits DHF reductase for plasmodium synthesis of DNA bases
Pyrimethamine side effect
Anti-folate that causes anaemia and thrombocytopenia. Cannot be given for seizure history.
Griseofulvin
Given for epidermal fungal infections such as tinea pedis, dermatophyte and tinea capitis. Inhibits fungal cell wall synthesis and binds to microtubules for nucleic acid synthesis. Binds to keratin in humans and microtubules.
Griseofulvin considerations
Cannot be given for acute porphyria and systemic lupus. Causes diarrhoea and GI issues.
Flu vaccines inactive
QIVc: cell propagation
QIVe: egg propagation
QIVr: recombinant to isolate HA gene
aQIVe: adjuvant added to egg propogated virus
Flu vaccine consideration
Called QIV for four strain protection of influenza virus. Given to infants over 6, pregnant women and immunocompromised. Live vaccine cannot be given to pregnant women
MMR vaccine
Live combination vaccine for pregnant women and children above 1 and adolescents. Requires more than one dose.
