What supportive management is provided for Transposition of the Great Arteries (TGA)?
Maintain body temperature, correct acidosis and hypoglycaemia if present.
What is the role of Prostaglandin E1 infusion in TGA management?
Maintains the ductus arteriosus.
What is balloon atrial septostomy (BAS) in TGA management?
Involves inserting a catheter into the foramen ovale and inflating a balloon to create a large atrial septal defect, allowing blood returning from the lungs to flow to the right side of the heart and into the aorta. A bolus of heparin is given before BAS.
What is the definitive management for TGA?
‘Arterial switch’ procedure in the first 2 weeks of life.
What is used during the ‘arterial switch’ procedure for TGA?
Cardiopulmonary bypass machine.
What additional step is needed during the ‘arterial switch’ procedure for TGA?
The coronary arteries also need to be transferred to the new aorta.
