What is the initial management for necrotising enterocolitis (NEC)?
Stop enteral feeding and medications, TPN may be required if feeds stopped >24 hours, feeds stopped for 7 days for confirmed NEC cases.
What is the purpose of an NG tube in NEC management?
Used to drain fluid and gas from the gut, monitor hourly gastric aspirates.
What type of antibiotics are used in NEC management?
Broad-spectrum IV antibiotics covering both aerobic and anaerobic organisms, e.g., cefotaxime and vancomycin.
Why are IV fluids used in NEC management?
For cardiovascular support, may require addition of inotropes.
When is surgery indicated in NEC management?
If perforation or failure to respond to medical treatment.
What surgical procedure is performed in NEC?
Laparotomy with resection of necrosed bowel with either a primary anastomosis or a defunctioning stoma.
