What is the treatment for inguinal hernia in children?
Surgical repair; timing is based on whether there are complications or not.

If there are suspected complications (i.e. strangulation or bowel obstruction).

Arrange urgent referral to paediatric surgeon, preferably within 2 weeks.

Elective repair at 2-3 years of age if the umbilical hernia is large or symptomatic (> 1.5cm), or if there are intermittent symptoms of incarceration or recurring pain.
What is the procedure if an umbilical hernia incarcerates during observation?
It should be manually reduced with pressure and surgically repaired at the earliest opportunity.
What should be done if an umbilical hernia cannot be reduced?
An emergency operation is required.
What are the PACES tips for explaining hernia to parents?
Explain the diagnosis (due to immaturity of the gullet leading to food coming back up), reassure that it is common and usually gets better with time, breastfeeding: offer assessment and alginate therapy, formula: review feeding history, smaller frequent feeds, thickeners, alginate therapy.
What should be included in the safety net advice for hernia?
Keep an eye on the vomitus (if it’s blood-stained or green seek medical attention).
