What is the indication of Orlistat? (2)
Weight loss in adults (18+) who are overweight or obese (BMI > 28kg/m2.
Check BMI before supply.
Explain the effect of Orlistat. (1)
Blocks absorption of 25% dietary fat.
What is the dose of Orlistat? (1)
60mg TDS immediately before, during or up to 1 hr after each meal. If no meal, no fat miss tablet.
Should be with a low fat diet (aids weight loss and minimises GI s/e.)
What is the max tx duration of Orlistat? (1)
6 months
What is the referral symptoms for Orlistat? (7)
< 18 yrs / BMI < 28kg/m2
No weight loss after 12 weeks of tx.
Taking > 6 months.
Pregnant/breastfeeding
Rx: Amiodarone, Levothyroxine, Antiepileptics, Ciclosporin, Warfarin, Anticoagulants, Anti-diabetics,
Conditions: HPT, Hypercholesterolaemia, Kidney disease, Malabsorption, Cholestasis.
Rectal bleeding when taking.
What are the common s/e of Orlistat? (5)
GI including leadkage, flatulence, diarrhoea, urgency, fatty oil stools ( less evident if eating low fat diet)
What counselling is given to patients taking Orlistat? (3)
Take multivitamins at bed time as ADEK are fat soluble vitamins.
Barrier method if on contraception and persistent diarrhoea.
Not a quick fit counselled on diet and exercise. Weight loss targets: 0.5-1kg/week.
