What is peri-oral dermatitis?
Erythematous eruption of small papules and with a distribution primarily around the nose and mouth. May affect the skin around the eyes (periocular dermatitis). Not a type of dermatitis.
What are the main featured of peri-oral dermatitis?
Affects women of all skin types 20-50 yrs.
Trigger/causes = unknown.
Associated with use of topical steroids or retinoids on the face.
It can also be triggered by steroid tablets/inhalers.
Cosmetics and even fluoride have been linked.
Stress/anxiety also potential triggers.
What are the treatment options for peri-oral dermatitis?
Will often resolve with no tx over a period of weeks.
If not, ABx treatment is indicated.
Px must stop the use of any steroid cream/cosmetics (triggers)
Avoid picking/squeezing spots or pustules.
Wash their face with water only until condition resolves.
Use an unperfumed basic moisturiser on a regular basis for dryness.
If no resolution after safe care:
- Oral ABx can reduce bacteria and skin inflammation e.g. Doxycycline/Lymecycline. Alternatives include Clindamycin/Erythromycin.
- Milder cases may settle with topical ABx creams.
- Most px need to take ABx for 2-3 months
Review each condition and know the symptoms and tx options and when to refer px to GP/A+E or recommend pharmacy first.
