What does MHRA state the use of Methadone?

A

When co-Rx with BZP/BZP-like drugs, CNS depressant s/e = higher risk of sedation, respiratory depression, coma and death.

What are the monitoring requirements of Methadone? (2)

A

Signs of respiratory depression at initiation and any Rx changes e.g. dose adjustments or new interactions.

If methadone is co-Rx with BZP or BZP-like drug, the respiratory depressant effect may be delayed. Px are monitored at least 2 weeks after initiation or changes in Rx.

What is the risk of taking Methadone? (1)

A

Can cause QT-interval prolongation:
- ECG monitoring recommended in px with risk factors for QT-interval prolongation whilst taking Methadone:
- Hx of cardiac conduction abnormalities.
- Family Hx of sudden death
- Heart/liver disease
- Electrolyte abnormalities
- Concomitant tx with drugs that can prolong QT interval.
