Written on FP10PCD (pink)
Must contain the prescriber’s private CD identification number.
Original form sent to NHSBSA
Keep a copy of the Rx in the pharmacy.
NHSBSA monitors private Rx of CDs.
Other drugs which are not Schedule 2-3 CDs aren’t allowed to be Rx in this form.
N/A to hospital private Rx.
