5 - Opth - Acute loss of vision - Optic disc swelling, Optic atrophy, Vitreous haemorrhage, Macular haemorrhage, Causes of bilateral acute painless vision loss Flashcards
OD swelling - appearance
disc contour appears blurred/poorly defined and pink
3 causes of OD swelling
Optic neuritis
Papilloedema
Anterior ischaemic optic neuropathy
Optic neuritis - features - what is no disc swelling?
blurring vision RAPD red desaturation retrobulbar aching eye movements painful
no swelling + retrobulbar neuritis
optic neuritis - common associations
commonly related to MS, also infections TB Syphilis
Papilloedema - 5 features
bilateral disc swelling due to inc IOP,
headache,
transient blurring + enlarged blind spots,
Severe > splinter haemorrhages, exudates, cotton wool spots,
Eventual atrophy
Optic Atrophy -appearance? when occurs? causes what?
disc pallor from lost axons, glial proliferation and decreased vascularity
without OD swelling OR following OD/ swelling
causes APD
2 main causes of Optic Atrophy
Optic nerve disease
Primary retinal disease (retinal vv/aa occulsion)
Other causes of optic atrophy
retinitis pigmentosa, toxic retinopathy, syphilis
Different optic nerve diseases that may cause atrophy
optic neuritis, papilloedema, ischaemic optic neuropathy, primary open angle glaucoma, optic nerve compression
Vitreous haemorrhage - causes
neovascularisation
retinal tear
PVD
trauma
Vitreous haemorrhage - presentation
floaters, blurred vision, red tint to vision, dark streaks in vision, field defects, often worse in morning
Macular haemorrhage - causes
proliferative diabetic retinopathy
AMD
macroaneurysms
macular haemorrhage - presentation
scotoma, distortion
Three areas that can cause bilateral acute painless vision loss
chiasm, optic nerve, cortex
Chiasm causes of bilateral acute painless vision loss
pituitary tumours, pituitary apoplexy
