1 - PSY - Antipsychotics Flashcards
AP’s effective Tx for wide range of conditions..?
psychosis anxiety mood disorders insomnia rapid tranq n + v hiccups tics incl in tourettes
3 main Tx aims using AP’s in psychosis
alleviate +ve Sx and -ve Sx of psychosis
minimise SE’s including metabolic disturbances
Which APs give more EPSE’s
typicals
what are the 4 main EPSE’s
akathisia - profound inner restlessness
parkinsonism - usually after several days to weeks, clinically identical to idiopathic PD - tremor, bradykinesia, rigidity
acute dystonia - involuntary mm spasms giving brief sustained abnormal postures
tardive dyskinesia - abnormal invol hyperkinetic movements - potentially irreversible . eg head nodding, grimacing
What are the features of Metabolic syndrome
WEIGHT
We - waist expanded (central obesity)
ig - impaired glucose (+insulin resistance)
h - HTN + HDL low (+LDL high)
t - TG’s high
most APs increase risk of this
Neuroleptic malignant syndrome - when? what should be done?
most frequently when initiating treatment - can be any time
medical emergency - all APs stopped and senior physician involvement
Neuroleptic malignant syndrome Sx -
FALTER Fever ( hyperthermia) Autonomic instability Leukocytosis/Low pH (met acidosis) Tremor/Tachycardia Elevated creatinine kinase Rigidity of muscles
AP’s causing weight gain - ++++/++/-
++++ olanzapine, clozapine
++ quetiapine, risperidone
- aripirazole
AP’s causing sedation
++++/+++/++/+/-
\++++ clozapine \+++olanzapine \++ quetiapine \+ risperidone - aripiprazole
AP’s causing metabolic disturbances ++++/+++/+/-

What to check before commencing AP
FBC, U+E, LFT, PRolactin, HbA1c, fasting lipids, check ECG, weight and BP
