13 - Opth - Trauma - Penetration trauma + chemical injury Flashcards
PT -do what urgently
refer
PT - when to suspect this
history of tool/metal work
PT - Signs of full thickness penetration - 7
Hx (sharp/high velocity injury) distorted globe subconjunctival haemorrhage distorted iris/pupil iris prolapse hyphaema (blood in ant chamber) Seidels sign
PT - What is seidels sign
clear streak of aqueous in flourescein-stained area
PT - What not to do
DO NOT touch, manipulate or pad the eye - immediate surgery required
PT - serious risk of?
endophthalmitis - local and systemic AB’s
Chemical injury - which are more serious and why
alkali more serious - risk of penetrating injury
-acids coagulate protein and show slower penetration
CI - Signs of chemical injury
redness, pain, blurring, photophobia
cloudy cornea
CI - What to do immediately
irrigate eyes - local may be needed first, use clean water/normal saline, before Hx or exam
CI - what to check regularly
pH using litmus paper - both for comparison
CI - complications of chemical injury
corneal scarring
opacification
lid damage
