3 - ENT - Otology - Sensorineural deafness - Noise induced, Barotrauma + Perilymph fistula, Trauma, Ototoxicity, Herpes Zoster Oticus Flashcards
apart from the above mentioned in the title, what can also cause sensorineural deafness..?
menieres
Noise induced hearing loss - how? loss at what Hz?
sudden or prolonged exposure
typically loss at 4-6Hz
Barotrauma - what happens
sudden pressure change may cause damage to inner ear membranes
What does barotrauma lead to?
barotrauma leads to leakage of perilymph into middle ear
How does barotrauma present?
sudden hearing loss, vertigo, tinnitus
Trauma - which fractures hold greater risk? risk of what?
transverse fractures ( 20% of temporal fractures) hold greater risk of inner ear and facial nerve damage
Ototoxicity - from what?
gentamicin
aminogylcosides, metronidazole, diuretics, aspirin, cytotxics, sedatives, antidepressants
Ototoxicity - what is vital and why? how to do this?
prevention - usually irreversible
pre-asses renal funciton, serum level monitoring, use non-ototoxic agents if possible
Herpes zoster oticus - syndrome name? what does virus attack? mgmt?
ramsay hunt syndrome
VZV attacks spiral/vestibular ganglion in inner ear
early aciclovir
Herpes zoster oticus - Sx and signs
intense pain, vesicular eruption on auricle/external ear canal
may be followed by sensorineural deafness, vertigo, facial paralysis
