A
- only treat if INR is above 10 or patient is actively bleeding

A
1) Hold warfarin
2) vitamin K 2.5 mg or 5 mg PO

Management of supratherapeutic INR in bleeding patient
A
- Vitamin K IV 10 mg, slow infusion, repeat q12h if warfarin persistently supratherapeutic
- 4 Factor-prothrombin complex concentrate (PCC)
