Conditions ET can transform into
Myelofibrosis, AML
High risk definition
1) personal history of VTE OR major bleeding
2) over age 60
3) platelet count greater than 1.5 x 10 to the 9th
Management of ET
High risk = hydroxyurea + ASA
Low risk = ASA 81 mg daily
Management of pregnant ET patient
Aspirin throughout pregnancy (pregnancy is thrombogenic)
- can’t give hydroxyurea
Important sequelae of ET + incidence + clinical importance
Acquired vWF (20% of patients develop acquired vWF due to consumption of vWF multimers secondary to platelet activation and proteolysis) – this limits ability to use aspirin in treatment. You can prevent acquired vWF using cytoreduction.
Presentation of ET
- may be asymptomatic
- may have vasomotor symptoms, bleeding, or clotting
