CAR-T cell therapy Flashcards
Basic process
T-cells engineered/modified to recognize cancer cells –> grown and cultured –> lymphodeplete pt (prevent rejection) –> infuse into patients
Medication used for lymphodepletion
Cytarabine, cytoxan
most important SE’s to know about
- cytokine release syndrome
- neurotoxicity (can be delayed)
- HLH/MAS
- B-cell aplasia leading to hypogammaglobulinemia
- infections (during lymphodepletion)
- TLS
- prolonged cytopenia
Problem you don’t want to have if other providers are unaware of CAR-T
Patients get transfused high dose steroids and you kill engineered cells
vector delivery
viral
CAR-T products available

management of hypogammaglobulinemia resulting from CAR-T
IF infection –> IVIG
most common toxicity of cellular immunotherapy
CRS
variables involved in CRS grading
fever
hypotension
hypoxia
second line treatment of CRS
Anakinrah
first line treatment of CRS
tocilizumab
second most common SE of CAR-T
neurotoxicity
Neurotoxicity presentation
anything (seizure, HA, encephalopathy, delirium)
Term for encephalopathy stemming from CAR-T
Immune effector cell-associated encephalopathy
Outpatient CAR-T?
Depends on product
