Cause
A
induction therapy with ATRA OR arsenic trioxide

Clinical features
A
patient treated with ATRA (usually 10-12 days after starting) or arsenic + leukocytosis + (dyspnea, fever, pulmonary edema or infiltrates, effusions, weight gain, bone pain)

CXR with differentiation syndrome
A
diffuse interstitial pulmonary infiltrates

Natural history of differentiation syndrome
A
Can be readily reversed

Treatment
A
Dexamethasone 10 mg IV BID ASAP x 5 days
Plan to taper steroids over 2 weeks
