Antihypertensive
Pharmacologic classification
ACE inhibitor
Therapeutic effects and uses
Used to treat heart failure, hypertension, and acute myocardial infarction.
Mechanism of action
Inhibits the action ACE, thus preventing the conversion of angiotensin 1 to angiotensin 2. Thus blood pressure is dropped.
Adverse effects
Headache, dry cough, dizziness, orthostatic hypotension, rash
Black box warning
Fetal injury and death may occur when ACE inhibitors are taken during pregnancy
Contraindications
Patients with Hyperkalemia (too much potassium)
History of angiosdemia
Renal impairment= caution
Drug interactions
Indomethacin and other NSAIDs= decrease antihypertensive activity.
NSAIDs also worsen preexisting renal disease.
Antihypertensives and diuretics= synergistic hypotension
Potassium supplements and potassium-sparing diuretics= hyperkalemia
Herbal/food interactions
Hawthorn= additive hypotension
Treatment of overdose
Normal saline or vasopressors or hemodialysis
