Hormone, drug for dysfunctional uterine bleeding

Therapeutic effects and uses
It antagonizes the effects of estrogen on the uterus, thus restoring normal hormonal balance. Prevents ovulation, and causes a thick cervical mucus that is resistant to the passage of sperm.
Mechanism of action
The drug inhibits GnRH, thus preventing the LH surge and ovulation

PO, IM, subcutaneous, IV
Absorption
Readily absorbed from the GI tract or IM
Distribution
Crosses the placenta; secreted in breast milk; 90% bound to protein

Adverse effects
Breast tenderness and breakthrough bleeding and other menstrual irregularities. Weight gain, depresison, hypertension (HTN), nausea, vomiting, fluid retention, and vaginal candidiasis
Black Box Warning
Progestins with estrogens may increase the risk of stroke, DVT, myocardial infarction, pulmonary emboli, and invasive breast cancer
Contraindications/Precaution
Contraindications: history of thromboembolic disorders, pregnancy, lactation, preexisting breast, uterine, or vaginal cancer.
Precautions: hepatic impairment, psychic depression
Drug interactions
Serum levels are decreased by aminoglutethimide, barbituates, primidone, rifampin, rifabutin, topiramate
Herbal/ food interactions
St. John’s wort may cause intermenstrual bleeding and loss of efficacy
