Renal cell carcinoma
What are the 4 stages of RCC?
- Confined to the kidney
- Spread to perinephric fat
- Extension to the renal
vein, IVC or lymph nodes - Extension to near or
distant structures
What type of carcinoma accounts for 85% of renal cell cases?
Adenocarcinoma
What are the clinical findings of RCC? (4)
- Painless gross hematuria
- Uncontrolled
hypertension - Palpable mass
- Flank pain
What is the US appearance of RCC? (6)
- Irregular mass with echogenicity ranging from hypo echoic (larger mass) to hyper echoic (smaller mass)
- Focal bulge in renal contour
- Indistinct borders
- Hypervascular mass 5. Metastasis to the lung, liver, and long bones
- Extension into the renal vein and IVC
What are are 5 differential diagnosis of RCC?
- Adrenal tumour
- Abscess
- Focal pyelonephritis
- Adenoma
- Angiomyolipoma
What is another term for Wilm’s tumour?
Nephroblastoma
What are the risk factors associated with Wilm’s tumour? (6)
- Beckwith-Wiedemann syndrome
- Hemihypertrophy
- Sporadic aniridia
- Male prevalence
- Omphalocele
- 5 years of age or less
Is Wilm’s tumour more common in males or females?
Males
What are the clinical findings of Wilm’s tumour? (5)
- Palpable mass
- Abdominal pain
- Nausea/vomiting
- Gross hematuria
- Hypertension
What is the US appearance of Wilm’s tumour? (7)
- Predominately solid, well- defined renal mass
- Variable echo pattern
- Echogenic rim
- Occasional calcification
- 10% - Intramural vascular flow
- Displacement of the IVC and aorta
- Metastasis to the renal vein, IVC, liver, contralateral kidney, and lymph nodes
What are 3 differential diagnosis of Wilm’s tumour?
- Neuroblastoma
- Renal carcinoma
- Mesoblastic nephroma
What causes metastases of the kidney?
Primary malignancy of
the bronchus, breast, gastrointestinal tract, contralateral kidney, and non-Hodgkin’s lymphoma
What is the clinical findings for metastases?
Asymptomatic
What is the US appearance of metastases?
Multiple small bilateral masses of variable echogenicity
