- Mass can be traced to the inguinal canal
- Peristalsis may be noted

- Anechoic fluid collection lateral and anterior to the testis
- Strong posterior acoustic enhancement
- Thin scrotal wall when acute
- Diffuse wall thickening when chronic
- May demonstrate internal echoes or septations
