Elderly females
Unilateral headache and jaw claudication
Ass w/ polymyalgia rheumatica
Granulomas
Takayasu Arteritis
Asian females < 40 yo
“pulseless disease” b/c weak upper extremity pulses
Granulomas in proximal great vessels
Polyarteritis Nodosa
Renal and visceral arteries (GI) but spares lungs
Transmural fibrinoid necrosis
Kawasaki
Asian kids < 4 yo
Connjunctuval injection, rash (polymorphus –> desquamating), cervical lymph, strawberry tongue, hand/foot changes
Give aspirin (only time for kids)
Worst comp = coronary artery aneurysm
Buerger Disease
Intermittent claudication and gangrene in smokers
Segmental
Smoking cessation prevents progression
Granulomatosis w/ Polyangitis
AKA Wegener
Upper resp/ nasal & lower resp & renal
Granulomas in lung
c - ANCA
Microscopic Polyangitis
Lung, kidney, skin (palpable purpura)
p-ANCA
NO granulomas and NO nasopharyngeal involvement
Churg-Strauss
AKA eosinophilic granulomatosis w/ polyangitis
Asthma, sinusitis, purpura, wrist drop, and GRANULOMAS
Inc IgE
p-ANCA
Henoch - Schonlein
Most common childhood systemic vasculitis
Often after URI
Triad - palpable purpura on butt + arthralgias + ab pain
IMMUNE COMPLEX MEDIATED
