Type 1

A

Distal renal tubule

Defect in ability of alpha intercalated cells to secrete H+

Inc risk of calcium phosphate stones in the basic urine

Hypokalemia because acidosis

Causes - amphotercin b toxicity, analgesic nephropathy, congenital anomalies

Type 2

A

Proximal tubule

Defect in bicarbonate reabsorption so inc bicarbonate excretion

Hypokalemia due to acidosis

Causes- fanconi, carbonic anhydrase inhibitors

Type 4

A

“Hyperkalemic”

Hypoaldosteronism leading to less K+ secretion - so less NH3 synthesis in proximal tubule and thus less NH4 excretion

Causes - dec aldosterone production (diabetic hyporeninism, ACE inhibitors, ARBs, NSAIDs, heparin, cyclosporine, adrenal insufficiency) or aldosterone resistance (k sparing diuretics, obstruction, TMP/SMX)
