what is vulvar squamous intraepithelial lesions (SIL)?
early superficial changes of the skin covering the vulva
*premalignant conditions
what is a low-grade vulvar SIL?
self-limited, resolve within 1-2 years
- occurs most often in condylomata acuminata
- no evidence of cancer precursor
- treat like condyloma: cryotherapy, topical meds
How is the dx made for LSIL?
Biopsy
what is high-grade vulvar HSIL?
HPV lesions, premalignant
* combined VIN 2 and VIN 3
who is vulvar HSIL more common in?
younger women 35-55
patients with vulvar HSIL commonly also have?
cervical intraepithelial neoplasia (CIN)
what is VIN, differentiated type?
hyperkeratotic plaque or warty papule or ulcer
who is VINd seen in?
older women 55-85 with lichen sclerosus, not HPV related
What is VIN differentiated type associated with/
Vulvar dermatoses
*lichen sclerosus
*NOT HPV related
What is the tx of VIN differentiated type
Biopsy
how do patients with vulvar SIL present? (Symptoms)
- itching or soreness, irritation
- burning
- dyspareunia
how do patients with vulvar SIL present? (Signs)
- localized lesions
- well-isolated
- found on hairless area
- color change: white, red, dusky
What is a common social history finding in vulvar SIL?
Smoking
