What is the cause of DeQuervain’s Tenosynovitis
A
- Cumulative disorder caused by overuse/repetitive movements of hand/wrist
*will be inflammation fo the thumbs extensor tendons
*Extensor pollicis brevis
*Abductor pollicis longus

Where does DeQuervain’s Tenosynovitis occur
A
Where the tendons cross the radial styloid
*tendon pass beneath the retinaculum and are surrounded by Tenosynovium

What are the risk factors of DeQuervain’s Tenosynovitis
A
- Employment
- Diabetes
- Inflammatory arthritis
- Pregnancy

What will be the presentation on PE?
A
- Radial wrist pain/burning
- May complain of popping and weakness
- Finkelstein Test (fist with thumb inside and ulnar deviation)
- First extensor compartment tenderness
- Thickening of tendon sheath
- Pain with resistance to thumb extension

What are the diagnostic findings?
A
Radiographs: maybe calcification
MRI: only if they failed conservative treatment

What are some of the complications?
A
- Can progress to tendinopathy or rupture
- Injection can cause risk discoloration, atrophy, tendon rupture, or elevated blood sugars

What is the treatment of DeQuervains Tenosnovitis
A
- NSAIDs
- Immobilization (thumb spica removable splint)
- Avoid offending activity
- Therapy
- Injection

What is the injection technique for DeQuervains Tenosnovitis
A
- Abduct thumb and Inject between the tendons, towards the distal radial styloid
- Use a numbing agent plus cortisone
*splint after injection

What is the incorrect way to complete the injection technique
A
- DO NOT inject directly into the tendon
- Do not inject if paresthesias present with needle insertion, redirect
