What are the two common types of hip fractures
- Femoral neck
- Intertrochanteric hip fractures
*in both the affected limb is generally shortened and in ER
What are femoral neck fractures due to?
Decreased bone mass and increased frequency of falls
*F>M
*elderly
What will a patient usually present with when they have a femoral neck fracture?
Osteoporosis
Where does the fracture occur within
The joint capsule
What is the clinical presentation of a femoral neck fracture
- Complaint of hip pain, with a h/o a fall
- Is the fracture is displaced they are usually unable to get up due to pain and instability
What are the PE findings
- Hip pain with any movement of the hip
- Affected side may be shortened
- Leg rests in external rotation
- Typically no significant swelling because fx is within the capsule
Where does the blood supply to the femoral head come from
Retinacular vessels
*the vessels will be damaged by the fax
*femoral head undergoes AVN
*will collapse with WB
What is the treatment of femoral neck fracture
- Hemiarthroplasty
- Early mobilization
After surgery what is the patient allowed to do
WBAT or Non WB for 6-12 weeks
*can attempt conservative non-operative treatment if fracture is stable/impacted
