How does a collies fracture happen?
A
- FOOSH
*mechanism causes transverse fracture of the distal radius just proximal to the wrist
*common in the elderly
*in a kids will present as a buckle or greenstick

What is the diagnostic test?
A
- X ray
*dorsal comminution and distal fragment impacted and shortened with apex volar angulation
*fracture is pointed towards palmar surface

How to reduce the fracture?
A
- Recreate hyperextension off the wrist
- Apply traction to the hand
- Force the wrist and distal fragment into flexion and ulnar deviation
- Cast (volar/dorsal splint from elbow to palm)

What can be used if there is misalignment
A
K-wire

What nerve can be injury during a collies fracture?
A
Median nerve injury
*document median nerve function before and after fracture
*tendon injury to thumb extensors is common as well

What is a common complication of collies fracture?
A
- Shoulder-hand syndrome (elderly)
*shoulder and finger stiffness results during treatment period
*patients should exercise joints while in cast
