What population has an increased risk of septic/infectious bursitis?
A
- Peds
- Immunocompromised

What causes Pre-patellar bursitis
A
- Overuse
- Direct blow to the area
- Chronic friction
- Crystal disease/gout

Where is the prepatellar bursa located?
A
- Anterior to the patella near the attachment of the patellar tendon

Is the prepatellar bursa superficial or deep?
A
- Superficial
*will reduce friction
*it is a flat synovial lined structure

What is the difference between Prepatellar bursitis and an effusion
A
- Prepatellar bursitis will swell out and around
*feel squishy - Effusion will be inside the knee and feel firm

How will a patient present with Prepatellar bursitis
A
- Pain and swelling over anterior knee

What are the PE findings>
A
- Swelling anterior to the patella (sponge like)
- Tender to palpation
- Decreased knee flexion

What are the labs to order?
A
- Radiographs
*for underlying fracture - CBC, protein, crystals, gram stain
* Prepatellar bursitis is a common place of infection

What is the treatment?
A
- RICE
- NSAIDS
- Aspiration with or without cortisone (effective)
- Surgical excision (failed conservative treatment)
