Diabetes Pharmacology Flashcards
when do glucose and insulin levels peak after a meal?
around 60 min
which pt has to secrete more insulin in order to maintain stable glucose levels?
obese pts
how long do you try a therapy before changing it, typically?
2-3 months
what are the primary goals of diabetic therapy?
- reduce risk of acute and chronic complications
- foster adherence
- limit side effects
- limit cost
when using dual or triple therapy, what should you do?
choose two drugs that do different things
what is a common first line med for DM?
metformin
when does metformin cause lactic acidosis?
drug is excreted in whole by the kidneys and if it hangs around, it will inhibit hepatic metabolism of lactic acid (60% mortality)
what is glucagon like peptide?
hormone released by the gut in response to glucose ingestion
what does glucagon-like peptide do?
stimulated insulin synthesis and secretion
suppresses glucagon secretion
reduces islet cell apoptosis
how is GLP 1 broken down?
DPP-IV
how do GLP levels change in type II diabetics?
decreased levels
what does DPP-IV do?
inactivates GLP 1
