The Form P-0004 (Intradepartmental Correspondence) should include a brief description of the circumstances behind the receipt of the award and a picture of the award itself. The Meritorious Awards Board shall not review the award for its merit, as that was done by the awarding agency, but rather to ensure the award conforms to uniform appearance guidelines as outlined in Order 401 (Dress and Personal Appearance). Upon approval, the member shall receive documented approval for daily wear on their Class A and Class B uniform. The Public Relations Executive shall also send a copy of the approval to the Human Resources Division and the employee’s chain of command.
