A. Name and Status Changes
1. Any member changing their name or marital status is required to report to the Human
Resources Division in person within ______________ after the change with a copy of the new Social Security card (or the application receipt for a new Social Security card) and to complete a new Department of Homeland Security, U.S. Citizenship and Immigration Services (USCIS) Form I-9 (Employment Eligibility Verification). The member must deliver the original documents supporting the changes (e.g., birth certificate, marriage certificate, divorce decree, legal guardianship, adoption papers, death certificate, etc.) to City of Jacksonville (COJ) Employee Benefits (City Hall) in order for the member’s record to be updated.

- Any employee changing their name shall report to the public counter at the Police Memorial
Building (PMB) to be electronically fingerprinted under the new name within 30 calendar
days after the change is made.

additions, or death of a dependent family member which will result in a change of benefits must be reported to the COJ Employee Benefits staff, (904) 255-5555, within__________________ of the event. c. Employees with assigned vehicles shall forward an updated Form P-0143E (Contract for
the Assigned Vehicle Program) to the Fleet Coordinator’s Office, via interdepartmental
mail or in person within one work cycle of returning to work after the change.
- The Communications Center shall not be called for employee telephone numbers, except between midnight and 0600 hours in emergency situations. The ____________________ shall handle all other inquiries and only for emergency situations, or when EIS is unavailable.
