Assessment
Data collection - gather an initial piece of info
Verification - Is the info accurate, Can I confirm/correlate that finding with another related finding?
What questions should you ask yourself when Analyzing? (6)
- What is going on with the patient? -
- What do these assessment findings mean?
- Why is this occurring, what is causing this to happen?
- HOW SICK IS MY PATIENT?
- how are they likely to deteriorate?
- What do I need to prioritize?
What do nurses generally do in the planning process? (7)
- set patient-centred goals
- outline expected outcomes
- plan interventions
- What should be avoided or is contradicted
- written plans of care
- prioritization
- evaluate evidence, applying relevant knowledge, problem solving
There are 4 different priorities for planning nursing care Name all 4.
Priority 1: Life-threatening situations
Priority 2: Safety Issues
Priority 3: Client priorities (what do they want?)
Priority 4: Nurse priorities (What is needed to be done?)
What is the implementation process?
-phase initiatives or completes planned action or nursing interventions
ex. looking up lab results, consulting physician, not directly done in front of patient
What are different ways of implementation? (3)
- direct vs indirect care
ex. at bedside, giving them meds - Independent vs dependant nursing interventions
ex. mobilizing patient, give bath without order from doctor, etc. - Medical orders
What is the step by step way of implementing?
- Re-assess patient
- Review existing nursing care plan
- Organize resources and care delivery
- Anticipate and prevent complications
What does the nursing practice of evaluation determine?
- determines effectiveness of nursing care
What are the 5 elements of EVALUATION?
- Identifying evaluation criteria and standards
- Collecting data to determine whether the criteria or standards are met
- Interpreting and summarizing findings
- Documenting findings and any clinical judgment
- Terminating, continuing, or revising the care plan
