Nurses provide safe, compassionate, competent and ethical care
How do we provide safe patient care?`(4)

Safety science is an informing approach to client safety. What is it, and why do we have it?
- reporting and responding to errors
- errors can lead to injury or death
What are the 4 TYPES of errors?
- Diagnostic errors
- treatment errors
- Preventive errors
- Communication errors
What are diagnostic errors? (4)
- the result of a delay in diagnosis
- failure to employ indicated tests
- use of outdated tests
- failure to act on results of monitoring or testing
What are treatment errors? (4)
- occurs in the performance of an operation, procedure, or test
- occurs in administering a treatment
- dose or method of administering a drug
- avoidable delay in treatment or in responding to an abnormal test
What are preventive errors?
- occur when there are failure to provide prophylactic treatment and in adequate monitoring for follow-up of treatment
ie. give drug but do not follow up
what are communication errors?
lack of communication or a lack of clarity in communication
- ie. wrote in the wrong chart
What is an error of commission?
- did not provide care correctly
What is an error of omission?
Did not provide care
What are the 3 different levels of error?
- Adverse event - no consequence
- Near Miss
- Sentinel Event - altered life or death due to error
How can we have an informing approach to client safety in terms of our environment? (2)
- Accountability - individual and organization/system level
- safe work environments, adapting systems, workplace culture
