How does it work? (3)
A
- bacteria use folate for DNA synthesis
- Trimethoprim inhibits bacterial folate synthesis
- this consequently slows bacterial growth - bacteriostatic

What type of bacteria does trimethoprim work against?
A
- gram positive and gram negative bacteria
e. g. E.coli (gram -ve bac)

- UTIs

What is the combination of trimethoprim and sulfamethoxazole used for?
A
Pneumocystitis pneumonia in the immunocompromised

Contra-indications (6)
A
- 1st trimester of pregnancy
- patients with folate def
- renal impairment
- elderly
- neonates
- HIV

Why can not trimethoprim be used in the 1st trimester of pregnancy?
A
- fetal abnormalities such as CVS or oral clefts
- due to folate antagonist

- GI upset
- nausea
- vom
- sore mouth
- Skin rash
- severe hypersensitivity reaction
- haematological disorders
- hyperkaelemia

- potassium elevating drugs
- aldosterone antagonists
- ACEIs
- ARBs
- Folate antagonists
- methotrexate - Drugs increasing folate metabolism
- phenytoin - Enhances the anticoagulant effects of warfarin
- kills the normal gut flora that synthesises vit K
