How does it work?
A
- lower blood glucose
2. stimulating pancreatic beta cell membranes to produce insulin

How do sulphonylureas stimulate the pancreatic beta cell membranes to produce insulin? (5)
A
- block ATP dependent K+ channels in the pancreatic beta cell membrane
- this causes depolarisation of the cell membrane
- the Ca2+ voltage gated channels open as a consequence
- intracellular Ca2+ increases
- this stimulates insulin secretion

A
- type 2 diabetes

Contra-indications (3)
A
- hepatic impairment
- renal impairment
- people with an increased risk of hypoglycaemia

- GI upset

- other anti-diabetic drugs
- drugs that increase glucose
e. g. prednisolone, thiazides, loop diuretics
- reduces the efficacy of gliclazide
