Sodium Bicarbonate Mechanism

A

Medscape:
Bicarbonate reacts with H+ ions to form water & carbon dioxide. It acts as a buffer against acidosis by raising blood pH

CT Protocols:
A buffer used in acidosis to increase the pH in cardiac arrest
Sodium channel blocker overdose to increase the sodium gradient across the cell membrane

Capital:
Metabolic acidosis during cardiac arrest, Tricyclic antidepressant overdose,

Sodium Bicarbonate Side Effects/ Precautions/ Comments

A

Capital:
Hypernatremia, Metabolic alkalosis
Deactivates sympathomimetics
Decreases effects of benzodiazepines

Repeat as needed in tricyclic antidepressant overdose until QRS narrows
Use in caution with fluid-restricted patients due to high sodium content

Poisoning/Overdose/Substance Use Disorder
Tricyclic with symptomatic dysrhythmias, (eg.
tachycardia and wide QRS):
* 2 mEq/kg IV/IO.

Cardiac Arrest
* 2 meq/kg IV/IO.
