Alteplase Mechanism

A

Medscape:
Recombinant human tissue-type plasminogen activator (t-PA); produces local fibrinolysis. Promotes thrombolysis by converting plasminogen to plasmin; plasmin degrades fibrin and fibrinogen

Capital:
Binds to fibrin-bound plasminogen at the clot site to convert plasminogen to plasmin. Plasmin digests the fibrin strands of the clot.

Alteplase Contraindications

A

Capital:
Active internal bleeding, Surgery within 2-3 weeks, Previous CVA, Prolonged CPR,
Brain or spinal surgery within 3 months, Brain cancer, Aneurysm, Recent significant trauma (especially head trauma), Uncontrolled hypertension (>185/110)

Alteplase Side Effects/ Precautions/ Comments

A

Capital:
Intracranial bleeding, Headache, Reperfusion dysrhythmias, Chest pain, GI bleeding,
Nausea/vomiting, Abdominal pain, Hypotension

Aspirin, heparin and other anticoagulants may increase risk of bleeding

Closely observe patient for signs of bleeding, Only administered via IV pump, Invasive procedures (IV starts, IM/SQ injections, etc.) should be avoided during administration
