Class II antidysrhythmic; beta-blocker
Metoprolol Mechanism
Medscape:
Blocks response to beta-adrenergic stimulation, cardio selective for beta1 receptors at low doses with little or no effect on Beta2 receptors.
Capital:
Blocks β-1 sympathetic receptors to decrease heart rate, contractile force and blood pressure.

Metoprolol Indications
CT Protocols:
Tachycardia
Capital:
Atrial fibrillation and atrial flutter with rapid ventricular response in patients taking PO β-blockers.
Supraventricular tachycardia refractory to vagal maneuvers and adenosine
Metoprolol Contraindications
Capital:
Bradycardia, High degree heart block, Hypersensitivity, Hypotension

Weakness, Bradycardia, Heart block, Hypotension, CHF/pulmonary edema, Wheezing, Shortness of breath, Nausea, Dizziness, Syncope
Metoprolol Adult Dose
CT Protocols:
Tachycardia
5 mg IV/IO over 2 – 5 minutes. May repeat every five minutes to a maximum of 15 mg as needed to achieve a ventricular rate of 90 – 100.
Capital:
5 mg IV/IO every 5 minutes; max 15 mg
