Ketamine Mechanism?
Capital: Binds to NMDA receptors to inhibit the action of glutamate and thus slow transmission of neural impulses; binds to opiate receptors to provide analgesia; mimics a sympathomimetic drug to increase hemodynamics and cause bronchodilation
Ketamine Indications?
CT Protocols:
Extreme agitation/combativeness, Suspected excited delirium, Ineffective control of agitated patient after benzodiazepines, Moderate to severe pain, Sedative used in Rapid Sequence Intubation, Post-intubation analgesia, Post-intubation sedation
Ketamine Contraindications?
CT Protocols: Hypersensitivity, significant hypertension
Ketamine Side Effects/Precautions?
Capital: Hallucination, Increased skeletal muscle tone, Increased salivation, Hypertension, Nausea/vomiting, Tachycardia, Palpitations
Potentiated by other sedatives
Reemergence reactions should be medicated with benzodiazepines.
Ketamine Adult Dose?
CT Protocols:
Induction for RSI: 2 mg/kg IV/IO
Restraints: 4mg/kg IM (round to nearest 50mg). Max single dose 500mg, may administer additional 100mg IM in 5-10 mins
1 mg/kg IV/IO over 2 minutes; may administer additional 0.5-1 mg/kg IV/IO in 5 minutes (max total 2mg/kg).
Pain management: 0.2 mg/kg IV/IO (Max 20mg/dose); Dilute dose in at least 10mL; Syringe push over 2-3 minutes or infusion, may repeat once in 10 minutes if indicated
Ketamine Pediatric Dose?
Capital:
Induction for RSI: 1-2 mg/kg IV/IO
Excited delirium: 3-4 mg/kg IM
Pain management: 0.3 mg/kg IV/IO/IM
Ketamine Onset of Action?
Medscape: 30 seconds IV; 3-4 minutes IM
Ketamine Duration of Action?
Medscape: 5-10 minutes IV; 12-25 minutes IM
