Diltiazem Mechanism

A

Medscape:
Nondihydropyridine calcium-channel blocker: Inhibits extracellular calcium ion influx across membranes of myocardial cells and vascular smooth muscle cells, resulting in inhibition of cardiac and vascular smooth muscle contraction and thereby dilating main coronary and systemic arteries; no effect on serum calcium concentrations; substantial inhibitory effects on cardiac conduction system, acting principally at AV node, with some effects at sinus node

Blocks calcium channels causing relaxation of vascular smooth muscle and slowing of conduction through the AV node.

Diltiazem Contraindications

A

CT Protocols:
Patients with heart block, ventricular tachycardia, WPW with atrial fibrillation, and/or acute MI.

Capital:
Hypotension, Sick sinus syndrome, High degree heart block, Bradycardia

Diltiazem Adult Dose

A

CT Protocols: Updated w 2023.1
Irregular tachycardia or regular tachycardia persisting after adenosine, consider:

10 mg IV/IO over at least 2 minutes. May repeat every 10 minutes, to a maximum total of 30 mg, to achieve a ventricular rate of <110.

Consider maintenance infusion at 5 – 15 mg/hour.
