Hydrocortisone Mechanism

A

Medscape:
Glucocorticoid; elicits mild mineralocorticoid activity and moderate anti-inflammatory effects; controls or prevents inflammation by controlling rate of protein synthesis, suppressing migration of polymorphonuclear leukocytes (PMNs) and fibroblasts, and reversing capillary permeability

Capital:
Inhibits pro-inflammatory substances and synthesis of pro-inflammatory enzymes

Systemic fungal infection, Premature infants, Hypersensitivity

Hydrocortisone Side Effects/ Precautions/ Comments

A

Capital:
Headache, Vertigo, CHF, Hypertension, Fluid and salt retention, Nausea

Solu-Medrol preferred for reactive airway disease and anaphylaxis; Solu-Cortef preferred for adrenal insufficiency

Incompatible with heparin

Give to any pt with known adrenal insufficiency if they also have:
Shock, fever, trauma, drowning, hyper/hypothermia, long bone fractures, vomit/Diarrhea w/ dehydration, respiratory distress, 2nd or 3rd degree burns, RSI
